Overview - MyChart is an Internet-based service that allows authorized users to access certain medical information and services on-line. Akron Children's Hospital offers MyChart as a convenience and courtesy to our patients and their families. MyChart does not display all medical information from your or your child's medical record, and should not be used as a complete medical record.

Eligibility - Unless otherwise allowed by law, an individual must be age 18 or older to request a MyChart account. Parents or guardians of minor patients may request access to a minor patient's health information by following the terms outlined in the "Proxy Access" section of this statement. Under State and Federal law, there may be circumstances where parents and guardians are not allowed access to a minor's health information.

To request an account, discuss your interest in MyChart with the physician at your next scheduled appointment.

Participation in MyChart is voluntary and you may deactivate an account at any time. Children's reserves the right to deactivate or prevent access to any and all MyChart features at any time for any reason.

Non-Emergency Only - MyChart services should never be used for emergencies or urgent situations. In an emergency, go to the nearest emergency department or call 9-1-1. For urgent needs, call your physician's office.

Response to Electronic Communication - We will review and respond to your message based on your health history and the information you provide. You will receive an e-mail when a response is available to you in MyChart.

Proxy Access - Parents and legal guardians may request proxy access to MyChart for their minor child by completing the Proxy Access Request Form. Proxy access will only be granted to those with legal authority to access the minor patient's information. If the proxy's legal relationship with minor patient changes, the proxy must inform Children's immediately. Children's may revoke proxy access at any time for any reason. Proxy access is also available for persons (such as legal guardians) who have legal authority to access an adult patient's information.

Proxies will have limited access to information regarding minors who are 12 years of age or older, in accordance with State and Federal privacy laws.

E-mail Privacy - MyChart allows users to send and receive secure electronic messages with Children's staff. Messages regarding medical issues will usually become part of the patient's medical record and will be available to those who participate in the patient's care and treatment. Users of MyChart will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. Thus, you should take this into account when providing an e-mail address to Children's.

Web Site Links - MyChart may offer links to related medical Web sites not managed by Children's. These Web site links are for informational purposes only. Children's does not endorse and has not verified the accuracy of the information on these Web sites, and you should not rely on any of the information found on these Web sites for purposes of treatment or diagnoses.

Web Site Availability - Children's may revise, change or update the Children's MyChart Web site periodically. The Web site may not always be available due to system maintenance, backups of failures.

Security and Confidentiality - We afford the same degree of confidentiality to health information stored on MyChart as is given to health information stored by Children's in any other medium. Children's is committed to protecting the confidentiality of your health information. We limit Children's employees' and medical staff members' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time you access MyChart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4). User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
